Uniforms:
Company provided uniforms, shoes, and safety glasses.
Effective after 91st day provided at low premium cost to employees:
Health Insurance: Employee and Family Coverage; $250
deductible per year; 80% paid by insurance after deductible for
in-plan doctors; prescriptions are paid 80% after deductible.
Dental Insurance: Employee and Family Coverage; $25
deductible per plan year, per family member. 100% Coverage twice per
year cleanings & fillings; other 50%
LTD: Long term disability; 60% of salary up to age 67
(coordinated benefit).
Life Insurance: For the employee in the amount of 1 times the
annual salary (max $50K) Optional additional Group Voluntary Life
Insurance is available
Accidental Death & Dismemberment Insurance: For the
employee coverage paid at 1 times the employee's annual salary (max
$50K)
Salary Continuance: All employees are salaried.
Holidays: 8 days of Holiday pay per year fixed
Effective after 1 year of employment:
401K Retirement Plan: Company contribution of 3% of salary
per pay period; employee may contribute up to 15% per pay period
through payroll deduction; Company will contribute $.35/dollar of
what employee contributes up to 5%. Merrill Lynch is investment
provider.
Paid Vacation: 1 Week after one year of employment; two weeks
after two years; three weeks after ten years; four weeks after
twenty years
